Abstract

An observational, analytical, cohort, retrospective-prospective study of clinical characteristics and embryological parameters of in vitro fertilization programs in women with unexplained infertility (93 women, 108 in vitro fertilization programs) in comparison with patients with tuboperitoneal infertility (45 patients, 49 in vitro fertilization programs) was conducted. The “clinical portrait” of a woman with unexplained infertility was described and its features were emphasized in comparison with patients with tuboperitoneal infertility. The frequency of blastulation rate in in vitro fertilization programs was considered as an end point, which was statistically significantly lower in the group of unexplained infertility, which is a sign of early embryogenesis disorder and, therefore, an indication for earlier in vitro fertilization in order to achieve pregnancy. In this case, it is inexpedient for couples with unexplained infertility to perform extended diagnostic examination, empirical treatment, fertilization by intracytoplasmic sperm injection into the oocyte, and preimplantation genetic testing for aneuploidy. The results of a pilot study – whole exome sequencing of the most typical couples with unexplained infertility are also presented.
